AfyaPro Connected Care App is the eHealth application to support people to better control their diabetes and hyper tension condition. It has a logbook for key measures like blood sugar, weight, blood pressure, insulin dosage and so on. A unique feature is the direct linkage of the App with the electronic health record of the health facility. The App has a chat function that allows direct and secure communication between the patient and the health professional. The App also shows carepaths, educational material or nutritional advice assigned (if any) by the primary care provider or health professional through the electronic health record of the health facility to the patient.
